Become a Collection Site

New DNA Collection Site Application

To become a Collection site for Health-Link and be added to our database, simply complete the following form and it will be sent directly to us by email. Your information will then be added to our database and we will contact you whenever we need collection services in your area. Keep in mind, collection sites are selected based on proximity (zip code) as well as price. The typical collection site charges $25 per person to collect samples.

The sample collection process consists of:

  1. Verify photo I.D.
  2. Complete one Chain of Custody form per person
  3. Collect 4 oral swabs per person
  4. Take a photograph to be submitted with the samples (the photo can be Instant- Polaroid or Fuji type or digital and printed) preferably take ONE photo with all tested parties in the same photograph. Have each adult sign and date the photo.

The entire collection process typically takes about 15 minutes and the clients can pay you at the time of service.

My name is Melanie Blythe. I have over 20years Phlebotomy experience and 17 years doing paternity blood/buccal collections. I would like to become a Collection Specialist in this area if there is a need. I reside in Bowie,Md which is close to Washington DC and parts of Va and Md. PLease let me know if there is an application to fill out. I also own a polaroid camera,fax machine,and have access to the internet via my cell phone as well.I look forward to hearing from someone soon.

I am a certified phlebotomist with over 20 years of experience and also an instructor.I am doing HIV/AIDS ora sure and rapid testing for over 10 years.I am working with a non profit
organization(Center for Multicultural Wellness and Prevention/CMWP),and I do have also
my own office(Gabriel Traffic School).I do speak,write,and read fluently:English,Spanish,
French and Creole which give me the opportunity to reach and serve different ethnicities.
